In the Multicenter Osteoarthritis Study (1,018 participants followed 84 months with MRI), quadriceps weakness increased the risk of worsening lateral patellofemoral cartilage damage in women (risk ratio 1.50 for the lowest versus highest strength quartile), a sex-specific and compartment-specific relationship that helps explain why strengthening programmes are compartment-relevant rather than generic.
